CREDIT REPORT AUTHORIZATION

I authorize Nationwide Finance Group, Inc. to order a credit report to determine if I pre-qualify for any loan   programs.      

I further authorize Nationwide  Finance Group,  Inc.  to verify my past and present employment earning records, bank statements and any other assets balance that are needed to process my prequalification for loan options that I/We may  qualify  for.

Answering the  question  below  will  serve  as my/our  authorization.
